Job Summary
Care Management Extender role supporting Tailored Care Management for behavioral health and medical care. Responsibilities include managing high-volume inbound/outbound member calls, coordinating care with providers and community resources, helping develop and update individualized care plans, linking members to housing, food, and social services, ensuring providers are up to date with tests/results, and facilitating ongoing communication among members, families, and treatment teams. Requires strong organization, time management, assertive engagement, and experience working in performance-driven environments with productivity and quality metrics. Qualifications include a high school diploma or equivalent, 18+, lived experience with behavioral health or related caregiving experience (peer support) or two years of relevant experience, onboarding completion, NC DHHS Tailored Care Management training within 60 days, and clean driving record. Compensation ranges from $17.36 to $22.29 hourly, with placement based on licensure, education, and experience. Locations include Raleigh, NC with hybrid remote options; E-Verify participant; equal opportunity employer.
Required Qualifications
- A high school diploma or equivalent; Must be 18 years or older; Be a person with lived experience with a behavioral health condition and a Certified Peer Support Specialist or a parent/guardian... with at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual or have two years of paid experience performing the functions described; Completion of onboarding training; NC DHHS Tailored Care Management training within 60 days; No driving infractions in 5 years
