Job Summary
Care Management Extender supports the Tailored Care Management team by linking members to healthcare and community resources, coordinating care across behavioral and medical services, educating members, developing individualized care plans, and maintaining communication among members, families, and providers. Responsibilities include outbound/inbound member calls, monitoring progress, assisting with care planning, connecting members to housing, food resources, and community programs, coordinating with providers, and ensuring up-to-date test results and information.
Required Qualifications
- A high school diploma or equivalent
- Must be 18 years or older
- Be a person with lived experience with a behavioral health condition and a Certified Peer Support Specialist or a parent or guardian of an individual with a behavioral health condition and has at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual
- Have two years of paid experience performing the types of functions described in the “Extender Functions” section, with at least one year of paid experience working directly with the Tailored Care Management eligible population
- Completion of general onboarding training upon hire
- Completion of NC DHHS identified training requirements for Tailored Care Management within 60 days of hire
- No driving infractions in the last 5 years
