Job Summary
Support and coordinate whole-person health care for behavioral health and medical needs; conduct regular outreach to members by phone and in person to assess needs, provide support, and monitor progress; connect members and families to healthcare services, housing assistance, food resources, and community programs; assist in developing and updating individualized care plans to support health and wellness goals; ensure continuity of care and timely follow-up among healthcare providers, members, and families; monitor for changes in symptoms and treatment side effects, and facilitate referrals and resources to improve member outcomes.
Required Qualifications
- A high school diploma or equivalent
- Must be 18 years or older
- Be a person with lived experience with a behavioral health condition and a Certified Peer Support Specialist OR a parent/guardian of an individual with a behavioral health condition and has at least two years of direct experience OR have two years of paid experience performing Extender Functions
