Job Summary
Senior LVN, Care Manager in the Outreach and Enrollment department coordinating care for outpatient patients. Responsible for engaging members, assessing their needs, and providing comprehensive care coordination to connect members with appropriate health programs and resources. Reports to the Manager of Care Management and supports the mission to improve health and wellness through proactive, collaborative care in the outpatient setting. Location: Rancho Cordova, CA. Strong emphasis on leadership, critical thinking, motivating teams, achieving results the right way, and ongoing learning.
Required Qualifications
- Licensed Vocational Nurse (LVN) license in California
- Experience in outpatient care coordination
- Ability to engage and connect members to health programs and resources
