Job Summary
Care Management Coordinator role focused on Chronic Care Management (CCM): conduct CCM outreach calls, review patient charts to identify care gaps, coordinate scheduling and referrals, follow-up on tests and provider recommendations, document all interventions in the EMR, ensure continuity of care after hospital or ED visits, and maintain detailed, patient-specific notes while complying with CCM program requirements. Requires active licensure/certification in a related healthcare field and 2+ years of healthcare/administrative clinical experience, strong communication, organizational, and EMR skills, and the ability to manage high-volume patient panels.
Required Qualifications
- Active licensure, registration, or certification in a related healthcare field (such as Nursing or Certified Clinical Medical Assistant) is required and must be maintained in good standing.
- 2+ years of experience in a healthcare or administrative clinical setting
- 2+ Experience with medical scheduling, referrals, or patient coordination
- Working knowledge with electronic medical records (EMR) systems
- Excellent communication skills (written, phone, email, and patient-facing interaction)
- Ability to work independently while following structured workflows
- Strong clinical judgment, chart review skills, and EMR proficiency
- Ability to manage high-volume patient panels independently
