Job Summary
Monitor electronic outreach dashboards to send and respond to patient text messages, conduct outreach to low-acuity patients, and perform wellness check-ins for chronic conditions like CKD and CHF. Upload discharge summaries and consult notes into partner EMRs, perform chart sweeps to identify triage encounters, and manage referrals and post-acute admissions. Mail patient communications including educational materials and Unable to Reach letters while completing administrative tasks to support operational efficiency. This role partners closely with Nurse Care Managers and clinical teams to support patient engagement and continuity of care for low-acuity populations. Must be located in Pacific or Mountain time zones.
Required Qualifications
- High school diploma or equivalent
- Must be located in the Pacific or Mountain time zones
- 2+ years of experience in care coordination, healthcare operations, medical records, or patient outreach
- Familiarity with chronic conditions such as CHF, CKD, diabetes, and hypertension
- Understanding of care management, transitions of care, and patient engagement workflows
- Experience working in EMRs/EHRs and healthcare technology platforms
- Proficiency with Microsoft Office or Google Workspace
- Experience handling inbound calls and using call center tools
- Strong written and verbal communication skills
- Ability to engage patients empathetically and professionally
- Strong collaboration skills
- Ability to manage high-volume, task-based work with competing priorities
- Ability to work independently while following standardized workflows
Desired Qualifications
- Associate's or bachelor's degree in healthcare, health administration, or a related field
- Licensed Professional Nurse (LPN) license
- Comfort working in evolving, early-stage or growth-oriented healthcare environments
