Care Management Coordinator (Full-Time, Day Shift)

Job Summary
Manage the discharge/transition process by working closely with patients and families, coordinating activities with physicians, nursing, and community resources to ensure adequate post-acute care. Compose and type confidential correspondence, reports, and materials from dictation, written, and verbal sources. Act as the discharge planning contact for patients and families during hospitalization while supporting physicians by providing necessary information to referral agencies. Coordinate small programs with limited budgets and serve as a working supervisor for team members. Perform other job-related duties as assigned, including learning processes to provide support when others are absent. Requires a high school education or GED, with an associate's degree or healthcare experience preferred. Full-time day shift position in Kings County.
