Care Management Coordinator (Duchess and Ulster counties)

Job Summary
Care Management Coordinator coordinates and participates in Care Management services for families and caregivers. Uses patient-centered motivational interviewing to build rapport and help patients improve health. Supports the primary care team with panel management to reduce loss to follow-up and non-adherence, and develops and adjusts individualized care plans for high-risk patients addressing medical and social barriers. Acts as a liaison among hospitals, primary care providers, specialists, and community resources to ensure patient-centered care coordination. Identifies and tracks special populations and those discharged from inpatient or emergency services. Uses team-based communication to close referrals and follow-ups, performing outreach across primary care sites, homes, hospitals, and neighborhoods.
Required Qualifications
- High School Diploma or equivalent required
- 1-3 years of relevant experience, required
- Bachelor's or Master's degree in relevant field strongly preferred
- Health home or care management experience strongly preferred
- NYS Driver's License strongly preferred
- Bilingual Spanish, preferred
