Care Management Coordinator

As a Care Management Coordinator on the CVS Health/Aetna DSNP care team, you will coordinate care for Dual Eligible members by collaborating with the care manager, social worker, and interdisciplinary team to address health, social determinants of health, and preventive care needs. Key duties include conducting the annual Health Risk Survey, identifying risks and service needs, completing care coordination activities within established timeframes, escalating quality or safety concerns, advocating for members, maintaining engagement through problem-solving and motivational interviewing, and ensuring accurate documentation in line with regulatory and accreditation guidelines. The role emphasizes collaboration, professional communication, and a strong commitment to member-centered care within Medicare and Medicaid managed care.
