Job Summary
Care Management Coordinator responsible for engaging children, youth, and families with significant behavioral health needs within the Care Management Entity. Performs High Fidelity Wraparound-focused care coordination (ICC/MCC), identifies care needs via CANS outcomes, develops and implements comprehensive care plans with networks of support, coordinates services across home, school, and community, ensures services are appropriate and least restrictive, links families to local resources, and maintains thorough documentation. Role emphasizes family-centered, strengths-based, trauma-informed, and culturally responsive approaches, with training in High Fidelity Wraparound required. Required Qualifications
- Bachelor's Degree human services field (preferred)
- Active driver’s license and proof of auto insurance
- Three years with a high school diploma or associate's degree
- Two years with a bachelor’s degree (human services field)
- Master's degree (human services field)
