Job Summary
Care Management Coordinator engages with children, youth, and families with behavioral health needs, providing intensive (ICC) and moderate (MCC) care coordination. Responsibilities include identifying care coordination needs aligned with High Fidelity Wraparound, developing and implementing care plans with networks of community supports, scheduling and conducting family/provider contacts, ensuring services are appropriate and least restrictive, linking families to local resources, documenting assessments and care plans, and building collaborative relationships with community partners while delivering culturally diverse, trauma-informed, and strengths-based care. The role emphasizes consistent communication, advocacy, and tailoring services to family needs across home, school, and community settings. Requirements include relevant degrees and licensure, training in wraparound models, and the ability to operate within a public-facing, mission-driven organization.
Required Qualifications
- Bachelor's degree in human services field (preferred)
- active driver’s license and proof of auto insurance
- Three years with a high school diploma or associate's degree
- Two years with a bachelor's degree (human services field)
- Master's degree (human services field)
