Job Summary
Care Management Associate (CMA) supports the ACC Case Management team with non-clinical care coordination and administrative duties to promote timely, effective member care. Remote, 40-hour weekly role serving CST/EST time zones, performing inbound/outbound member outreach, referrals processing, case tracking, documentation in clinical systems (e.g., MedCompass, CEC), and facilitating communication among interdisciplinary care teams and external stakeholders. Requires HS diploma/GED, 1+ year of healthcare-facing customer service experience (preferred), strong communication and organizational skills, medical terminology familiarity, and proficiency with Microsoft Office. Location flexibility via fully remote work across multiple US states; dedicated home workspace required. Anticipated learning and collaboration within CVS Health and the Aetna Clinical Collaboration program, with emphasis on compliant documentation and coordinated care planning.
Required Qualifications
- High School Diploma or GED (REQUIRED)
- 1+ year(s) of customer service experience (healthcare environment preferred)
- Strong customer service and member engagement skills
- Strong organizational skills, including effective verbal and written communication
- Basic knowledge of medical terminology
- Proficiency in Microsoft Office (Word, Excel, Outlook) and ability to navigate multiple systems
