Job Summary
Care Management Assistant in ED Navigation coordinating patient care with nurses and social work care managers; serves as liaison between department, payers, physicians, and families; handles referral status, medical records retrieval, regulatory notices, scheduling, and advance directive/notarization support; supports transition planning, community resource navigation, and care plan monitoring; requires clinic/hospital healthcare experience, strong communication, and current BLS and driving eligibility; part-time role located in Denver, CO with on-site requirements.
Required Qualifications
- Demonstrated healthcare experience in a clinic or hospital setting.
- Demonstrated customer service with a focus on communications and problem resolution.
- Proficiency in advanced computer skills.
- Caregivers whose duties require them to conduct home or community visits must maintain current BLS certification, have a current driver’s license, current auto insurance, an acceptable driving record and reliable transportation.
