Care Extender / Peer Support Specialist- Foster Care - Region 1 Haywood County

Engage with community partners and build strategy with local stakeholders to provide education, strengthen relationships, and resolve case or systems issues. Support care coordination teams by connecting members and caregivers with community peer support services and appropriate expertise to overcome challenges. Identify engagement opportunities for members, caregivers, and families to share with the care management team, while partnering with community services to create referral pathways. Support care managers in advocating for foster parents and caregivers by addressing their needs or challenges with various agencies and systems. This role requires a high school diploma, two years of related experience, and must be performed by a resident of Haywood County, NC, in a hybrid schedule with one to two in-office days per week.
