Job Summary
Utilize an integrated case management model to assess, plan, implement, and monitor member health while documenting observations and reporting behavioral concerns in clinical records. Monitor member safety and mental health symptoms, maintaining accurate records and updating service plans with time-specific actions. Act as an advocate and liaison to coordinate resources, communicate with interdisciplinary teams, and explore community options. Participate in required supervisions, complete training, and perform physical de-escalation techniques as necessary. Maintain confidentiality per HIPAA policies and adhere to organizational procedures.
Required Qualifications
- Bachelor's degree in a field related to behavioral health, social work etc.
- Associate's degree in a field related to behavioral health, social work etc. AND 2-years related experience
- H.S diploma and 4 -years behavioral health related experience
- Minimum 18 years of age
- DPS Level I fingerprint clearance
- CPR, First Aid, AED certification
- Current, valid Arizona Driver's License
- 39-month Motor Vehicle Report
- proof of vehicle registration liability insurance
- Initial current negative TB test result
- ability and mobility to perform physical de-escalation techniques
- ability to take and pass a physical de-escalation exam
- ability to stand or sit for prolonged periods of time
- ability to occasionally stoop, bend, kneel, crouch, reach, and twist
- ability to lift, carry, push, and/or pull a moderate amount of weight
