Job Summary
Respond to Healthy Start referrals in a timely manner, providing education and support in childbirth, breastfeeding, and parenting to pregnant women and families with children aged 0-3. Explain available services, identify risk factors, and develop cooperative relationships with primary health care providers to facilitate referrals to community resources such as WIC, food banks, and mental health services. Maintain updated progress notes and ensure all client records are accurate, current, and properly linked within the database system in compliance with Florida Healthy Start Standards and Guidelines. Manage a caseload of approximately 40 consumers per month while attending staff meetings and training as assigned.
Required Qualifications
- Graduation from an accredited four-year college with a degree in human services, social sciences, social work, nursing, health education, health planning, healthcare administration, or related field
- 2 years of public health/community development experience
- Must have reliable means of transportation
- CPR Certification
- First Aid Certification
- an annual Tuberculosis test
- Ability to utilize Microsoft Office Software Suite
- Ability to provide an Affidavit of Good Moral Character
- Clearance in Pre-Employment Drug Testing and Criminal Records Checks
Desired Qualifications
- Graduation from an accredited masters program with a degree in Social Work, Psychology or related field
- Bilingual
