Job Summary
Maintain an active caseload with monthly member contact, conducting home, community, and office visits to support engagement and service delivery. Lead interdisciplinary team meetings, coordinate communication among providers and partners, and connect members with healthcare, housing, employment, benefits, and transportation resources. Monitor progress, adjust care plans, and complete annual and 90-day assessments in compliance with DHHS and Behavioral Health Home requirements. Identify service gaps, collaborate to address barriers, and maintain accurate documentation while promoting self-advocacy and independence. Participate in quality improvement and audit initiatives, serving as a liaison among families, providers, and community agencies.
Required Qualifications
- Bachelor's degree in a related field
- MHRT/C eligible
- A minimum 1 year of paid experience working with individuals with behavioral health, intellectual and/or developmental disabilities (e.g., DSP, BHP, RBT, etc.)
- Strong organizational, writing, and time management skills
- Ability to thrive in a fast-paced environment with multitasking and problem-solving responsibilities
- Microsoft Office Suite

- Experience working with Behavioral Health Home Services
- Previous experience in care coordination
