Job Summary
Conduct introductory, supervisory, and ongoing client visits including 30-, 60-, 90-day, and quarterly assessments to monitor conditions and service quality. Develop, implement, and update individualized care plans while responding to incidents, hospitalizations, and condition changes within 24 business hours. Collaborate with Scheduling Coordinators to align caregiver-client matching and ensure caregivers are properly trained and prepared. Maintain accurate documentation in the company's home care software system and provide coaching to caregivers on performance and client satisfaction. Support the Business Development team with community outreach and events, occasionally assisting with direct care shifts during staffing shortages. Work approximately 60% office-based and 40% field-based, adhering to a Monday through Friday schedule with on-call rotation for evenings and weekends.
Required Qualifications
- High school diploma or GED
- Valid driver's license
- Reliable vehicle
- Current auto insurance
- Current CPR and First Aid certification
- Ability to travel throughout the service area during the workday
- Successful completion of background check and motor vehicle report
- Current negative TB test or chest x-ray
- Ability to safely perform job duties in both office and client home environments
- Ability to travel frequently within the service area
- Ability to remain professional and effective in fast-paced and high-pressure situations
Desired Qualifications
- College degree
- Minimum 2 years of care coordination or related healthcare experience
- Experience in home care, home health, assisted living, or similar setting
- Clinical background (e.g., CNA, LPN)
