Job Summary
Care Coordinator provides comprehensive care coordination for youth, adults, and families to promote access to integrated healthcare, behavioral health, and community-based services. Acts as a liaison between patients, families, providers, schools, community agencies, and other stakeholders; helps navigate complex systems of care; advocates for individual needs and goals. Uses a strengths-based, trauma-informed, culturally responsive approach to support patient engagement, service planning, resource linkage, care transitions, and multidisciplinary collaboration. Contributes to whole-person wellness and improved health outcomes through coordinated, integrated service delivery.
