Job Summary
Provide case management with crisis intervention, discharge planning, and group and individual work, including mental health and substance use disorder counseling. Recommend and assist access to medical, educational, social, vocational, and housing resources while coordinating with physical health providers for whole-person care. Complete accurate documentation, meet billable requirements, and facilitate placements with pre-placement visits and transportation. Maintain confidentiality, participate in program meetings, and provide field-based services as needed. Shift coverage includes part of the on-call team. This role supports TURN's mission of improving lives through client-centered care.
Required Qualifications
- Bachelor's degree
- four years of experience in social services
- Valid CA driver's license
- current auto insurance per contract requirements
- Cultural competency understanding
Desired Qualifications
- Three years of experience providing case management to individuals with mental illness and/or substance use disorders
- Experience providing mental health and/or substance use disorder services
- Bilingual
- Knowledge of Microsoft Office
- EHR software
