Job Summary
Conduct integrated case management utilizing assessment, planning, implementation, and monitoring to manage a member's whole health. Observe members, listen to concerns, and document behaviors or interactions in clinical records. Monitor health and safety, reporting signs of mental illness to appropriate staff. Act as an advocate and liaison to identify providers, explore community resources, and coordinate services with interdisciplinary teams and support systems. Develop time-specific service plans, maintain records with appropriate billing codes, and participate in required supervision. Transport members as needed and perform physical de-escalation techniques while adhering to all organizational policies and HIPAA regulations.
Required Qualifications
- Bachelor's degree in a field related to behavioral health, social work etc.
- Associate's degree in a field related to behavioral health, social work etc. AND 2-years related experience
- H.S diploma and 4 -years behavioral health related experience
- Minimum 18 years of age
- DPS Level I fingerprint clearance
- CPR, First Aid, AED certification
- Current, valid Arizona Driver's License
- 39-month Motor Vehicle Report
- proof of vehicle registration liability insurance
- Initial current negative TB test result
- ability and mobility to perform physical de-escalation techniques
- ability to take and pass a physical de-escalation exam
- ability to stand or sit for prolonged periods of time
- ability to occasionally stoop, bend, kneel, crouch, reach, and twist
- ability to lift, carry, push, and/or pull a moderate amount of weight
