Job Summary
Utilize an integrated case management model to assess, plan, implement, and monitor member care while managing whole health needs. Observe members, document behavioral concerns in clinical records, and monitor health and safety signs to report issues per policy. Provide services and coordination based on member requests, act as an advocate for resource access, and collaborate with interdisciplinary teams to develop individualized, time-specific service plans. Maintain member records, complete progress notes with appropriate billing codes, and participate in required supervisions. Transport members as needed and explore community resources for alternative options. Adhere to organizational policies, HIPAA regulations, and licensing requirements while maintaining an approved schedule.
Required Qualifications
- Bachelor's degree in a field related to behavioral health, social work etc.
- Associate's degree in a field related to behavioral health, social work etc. AND 2-years related experience
- H.S diploma and 4 -years behavioral health related experience
- Minimum 18 years of age
- DPS Level I fingerprint clearance
- CPR, First Aid, AED certification
- Current, valid Arizona Driver's License
- 39-month Motor Vehicle Report
- proof of vehicle registration liability insurance
- Initial current negative TB test result
- ability and mobility to perform physical de-escalation techniques
- ability to take and pass a physical de-escalation exam
- ability to stand or sit for prolonged periods of time
- ability to occasionally stoop, bend, kneel, crouch, reach, and twist
- ability to lift, carry, push, and/or pull a moderate amount of weight
