Job Summary
Connect with members, learn their stories, and help them improve their quality of life through individual, group-based, and community collaboration interventions. Perform crisis stabilization functions, provide face-to-face crisis intervention, and complete safety plans with consumers. Coordinate care by reviewing documents, assessing consumers per clinical protocols, and developing service plans based on diagnostic processes. Monitor mental status, update treatment plans, and transition consumers to appropriate levels of care. Assist consumers and guardians in accessing community resources, securing appointments, and navigating hospital discharges while maintaining close coordination with PCP health plans and external stakeholders. Conduct outreach for missed appointments, complete chart closures, and ensure quality and compliance in consumer records. Travel to the east or west valley once weekly, with the role mostly in-office.
Required Qualifications
- 21-year age
- Bachelor's degree
- one year of full-time behavioral health experience
- combination of experience and education totaling 5 years
- fingerprint clearance issued by the Arizona Department of Public Safety
- eligibility to receive fingerprint clearance within eight (8) weeks of hire
- drug screening
- drug-free workplace compliance
