Job Summary
Care Coordinator responsible for scheduling, caregiver support, and client satisfaction within a home care team. Key duties include coordinating caregiver schedules, responding to call-outs, maintaining scheduling software, and communicating with families. Drives sales and referral growth through community outreach and follow-ups with hospitals, senior centers, and referral partners. Involved in caregiver recruitment and onboarding, delivering orientation and ongoing training, and providing coaching to improve performance. Builds relationships with caregivers and clients to ensure high satisfaction and retention while supporting agency growth and adherence to state regulations and agency policies.
Required Qualifications
- Experience in home care, healthcare, or scheduling strongly preferred
- Strong interpersonal and communication skills
- Sales or community outreach experience a plus
- Ability to multi-task, prioritize, and work under pressure
- Comfortable with technology and scheduling software
- Training or leadership experience preferred
