Job Summary
Care Coordinator role at Sentara Health Plans in Williamsburg, VA. Responsibilities include performing a variety of casework duties and providing case management to patients, families, and designated caregivers; developing and monitoring multidisciplinary collaboration of services; assisting adult patients and families with personal and environmental challenges related to medical conditions up to terminal illnesses; conducting both in-person/face-to-face assessments and remote telephonic assessments for members with Serious Mental Illnesses, Serious Emotional Disturbances (ages 2+) and Substance Abuse Disorders; travel up to 90 minutes one way; standard hours 8am-5pm EST, Monday-Friday; requires an appropriate health & human services degree and relevant experience in long term care, Medicaid/health plans, and case management. Required Qualifications
- Bachelor's degree in Health & Human Services or related Fields as listed in the posting
- Long Term Care - 1 year
- Health Plan - 1 year
- Medicaid - 1 year (preferred)
- Case Management - 1 year (preferred)
- At least one year of mental health experience (preferred)
- Experience with Serious Mental Illnesses, Serious Emotional Disturbances, Substance Abuse Disorders (preferred)
- Education credentials must be from accredited U.S. institutions passed to standard accreditation databases found on the U.S. Department of Education site
