Job Summary
The Care Coordinator is an experienced clinical professional who provides guidance and education to patients and families. The role is responsible for health management and coordination of care for a designated population of patients, with a deep understanding of patients' medical needs, insurance coverage, health literacy, and other socioeconomic factors that may impact wellness. The coordinator aids patients in overcoming barriers presented by these elements and supports care planning and advocacy across the care continuum. The position is based in North Augusta, SC with part-time, weekend shifts, and requires an active nursing license and BLS certification, with preferred qualifications including a Bachelor's in Nursing and experience in case management.
Required Qualifications
- Associates Degree in Nursing
- 2 years or more of experience in Nursing
- Current American Heart Association or American Red Cross Basic Life Support (BLS)
- Current Registered Nursing License (Georgia) or qualifying multi-state compact license
