Care Coordinator RN - PT/Wknds - Augusta

Provide guidance and education to patients and families while managing and coordinating care for a designated population. Assess each patient's medical needs, insurance coverage, health literacy, and socioeconomic factors to identify barriers to wellness and assist in overcoming them. Requires an Associate Degree in Nursing with two years of experience, current BLS certification, and a valid Georgia RN license. Preferred qualifications include a Bachelor's Degree in Nursing, one year of case management experience, and National Certification in Case Management.
