Job Summary
Care Coordinator RN role focusing on health management and coordination of care for a designated patient population. Provides guidance and education to patients and families, considering insurance coverage, health literacy, and socioeconomic factors to enable wellness. Works with an interdisciplinary team to address barriers and ensure effective care transitions. Requires an active RN license (GA or compact), BLS certification, and an Associate Degree in Nursing; Bachelor's in Nursing and case-management certification are preferred. Day shift with part-time weekend coverage in Augusta area of Georgia, located at Belvedere, SC (per posting hints).
Required Qualifications
- Associates Degree in Nursing
- 2 years or more of experience in Nursing
- Current American Heart Association or American Red Cross Basic Life Support (BLS) certification
- Current Registered Nursing License (Georgia) or qualifying multi-state compact license
