Care Coordinator RN - PT/Wknds - Augusta

Provide guidance and education to patients and families while managing care coordination for a designated population. Assess each patient's medical needs, insurance coverage, health literacy, and socioeconomic factors to identify barriers to wellness and assist in overcoming them. Maintain current Registered Nursing License in Georgia and hold either American Heart Association or American Red Cross Basic Life Support certification. Requires Associate Degree in Nursing with two years of clinical experience; Bachelor's degree and case management experience preferred. Work full-time day shifts at Wellstar, the most integrated health system in Georgia.
