Job Summary
Care Coordinator RN working part-time with weekend shifts, providing guidance and education to patients and families. Responsible for health management and coordination of care for a designated patient population, considering health literacy, insurance coverage, and socioeconomic factors that may impact wellness, and assisting patients in overcoming barriers to care. Demonstrates clinical expertise and supports care teams in implementing individualized plans, with emphasis on patient education, collaboration with multidisciplinary colleagues, and adherence to standards of nursing practice.
Required Qualifications
- Associates Degree in Nursing
- 2+ years of nursing experience
- Current American Heart Association or American Red Cross Basic Life Support (BLS) certification
- Current Registered Nursing License (Georgia) or qualifying multi-state compact license
