Job Summary
Guide and educate patients and families on health management and care coordination for a designated population. Assess medical needs, insurance coverage, health literacy, and socioeconomic factors to help individuals overcome barriers to wellness. Provide daily support to registered nurses and maintain current Georgia licensure and Basic Life Support certification. This role is part of Wellstar's integrated health system in Augusta, Georgia, focusing on enhancing the health and well-being of every person served.
Required Qualifications
- Associates Degree in Nursing
- 2 years or more of experience in Nursing
- Current American Heart Association or American Red Cross Basic Life Support (BLS)
- Current Registered Nursing License (Georgia) or qualifying multi-state compact license
- Day (United States of America)
Desired Qualifications
- Bachelor's Degree in Nursing
- 1 year or more of experience in Case Management
- National Certification in Case Management
