Job Summary
Care Coordinator RN responsible for guiding and educating patients and families, coordinating health management for a designated patient population, and addressing barriers related to insurance, health literacy, and socioeconomic factors to support wellness. Requires an Associate Degree in Nursing, active RN license in Georgia or compact license, and BLS certification; preferred Bachelor's Degree in Nursing, experience in Case Management, and national Case Management certification. Day shift in Harlem, GA.
Required Qualifications
- Associates Degree in Nursing
- Current Registered Nursing License (Georgia) or qualifying multi-state compact license
- Basic Life Support (BLS) certification from American Heart Association or American Red Cross
