Job Summary
Coordinate care for neonate, adolescent, adult, and geriatric patients, families, medical staff, and community agencies to determine appropriate settings of care and hospitalization appropriateness. Assess post-hospital care needs and provide support to address social drivers of health. Requires a Georgia RN license, two years of clinical experience, and skills to interact with patients, families, physicians, and hospital staff. No weekend or on-call requirements.
Required Qualifications
- Graduate of an accredited Registered Nursing School (RN)
- Current License in the State of Georgia as a Registered Nurse or Nursing Compact (NLC/eNLC) or Multistate License
- Two (2) years previous clinical experience
Desired Qualifications
- BSN
- Three (3) years of experience in case management, social services or related field
- Certified Case Manager
- Ability to interact with patients, families, physicians and hospital staff
- Excellent oral and written communication skills
- Ability to type accurately at an approximate rate of 30 wpm
