Job Summary
Coordinate care for youth and families with complex behavioral needs through strength-based, individualized planning and team-based meetings. Conduct home, school, office, and community visits to assess, empower, and support clients while managing crisis situations and on-call duties. Lead Child and Family Team Meetings, facilitate Plan of Care implementation, and maintain HIPAA-compliant documentation and billing records. Deliver culturally responsive services across diverse settings to reintegrate children into family placements.
Required Qualifications
- Masters Degree from an accredited university or college with major course work in social work, psychology, and counseling or a closely related field
- at least two (2) years full-time experience in providing mental health services/social services case management or mental health counseling; services must be provided in mental health, community-based, or probation setting
- Experience in an academic counseling setting does not qualify
- excellent organizational, analytical, communications, and care coordination skills
- ability to empower people with a strength-based approach
- ability to prioritize responsibilities
- ability to build rapport with a wide variety of individuals
- Ability to meet professional standards and requirements for assigned duties and responsibilities
- Proficient with Outlook, Microsoft Word, Excel and PowerPoint
- Maintain a variety of online recordkeeping and documentation processes and systems for assigned clinical program
