Care Coordinator - Rehab at Complete Care focuses on supporting clinical care by delivering therapies safely, documenting in real time, and coordinating across administrative and clinical teams to optimize patient outcomes. Responsibilities include executing therapeutic protocols, monitoring patient tolerance, ensuring care plan adherence, distributing and training on Durable Medical Equipment (DME), documenting therapy services in the EHR, coordinating insurance and authorizations, and supporting billing integrity. The role emphasizes collaboration with Administrative Care Coordinators, providers, imaging teams, and cross-departmental staff to maintain compliant, high-quality patient care. Ideal candidates have strong communication, attention to detail, EHR proficiency, and prior rehab/therapy or patient-support experience (1-2 years preferred). Clinic hours are provided, and a Level 2 background check is required.
