Care Coordinator - Registered Nurse (RN)

Job Summary
Assess transitional care needs, coordinate care across the continuum, and engage with patients and families to assure care needs are met. Plan effectively to meet patient needs, manage length of stay, and promote efficient resource utilization by facilitating care progression and transitional care planning. Conduct psychosocial and functional status assessments, collaborate with interdisciplinary teams on discharge goals, and proactively identify barriers to expedite discharge. Manage all aspects of discharge planning, including insurance coverage verification, post-acute referrals, and consensus building with physicians and payers. Document clinical and psychosocial assessments, track avoidable days, and resolve diagnostic or treatment delays to ensure timely transitions. Complete mandatory education, serve as a preceptor, and comply with Wellstar Health System policies.
Required Qualifications
- Associates Nursing or Diploma (Nurse) Nursing
- Bachelors Nursing
- RN - Reg Nurse (Single State) or RN-COMPACT - RN - Multi-state Compact
- BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life Support or BLS-I - Basic Life Support - Instructor
- Minimum 1 year nursing experience in the acute care setting
- Excellent written and verbal communication skill
- Maturity, self-confidence, objectivity, and positive attitude
- Self-directed with the ability to function well under stress, handle change, and function in a fast-paced environment
- Strong assessment, interview, organizational and problem-solving skills
- Knowledge regarding local, state and federal regulations
- Knowledge of community and state-wide resources and programs
- Ability to work collaboratively with physicians, members of the care team, and the patient/family to assist with progression of care through their transition to the next level of care
- Must possess maturity, self-confidence, objectivity, and positive attitude
- Self-directed with the ability to function well under stress, handle change, and function in a fast-paced environment
