Job Summary

Conduct client intakes, assess needs, and develop care plans to promote client independence by overcoming barriers. Build strong client relationships, coordinate support with other service providers, and make referrals to internal and external resources. Facilitate client team meetings and ensure follow-up to achieve client goals. Manage client caseloads within assigned service programs while analyzing problems and developing solutions at a functional and strategic level. Requires a valid driver's license, reliable transportation, and ability to travel to outer regions as assigned.

  • An associate degree in a related field
  • 1 years of experience in a setting providing direct services to clients
  • Valid driver's license
