Job Summary
Care Coordinator for PICU/TICU engaging in collaborative care planning and coordination to meet complex pediatric patient needs. Responsible for assessing, planning, implementing, monitoring and evaluating care across the continuum, coordinating an interdisciplinary team, ensuring appropriate level of care, facilitating timely information flow, supporting families, and driving quality improvement and utilization processes. Partners with physicians to allocate resources, ensure patient and family education, support self-care, and optimize outcomes within a value-based care framework. Requires respiratory fit testing, nursing licensure, and BLS; seeks strong communication, customer service, and cross-functional collaboration skills to coordinate care across settings.
Required Qualifications
- Bachelor’s degree in nursing or associate's in nursing with enrollment in BSN program
- Current RN Licensure by the Texas Board of Nurses or Nursing licensure compact
- BLS from AHA
- 3yrs of nursing experience in an acute care setting
