Job Summary
Care Coordinator position focusing on coordinating and sustaining services for individuals with developmental disabilities and supporting their caregivers. Serve as the primary contact after services begin, build strong relationships with members and their support teams, attend PCSP meetings and advocate for member needs, ensure services align with member goals and authorized support, address questions and service issues promptly, monitor EVV compliance and documentation, assist with billing and authorizations, support caregiver recruiting, onboarding, and member intake, and maintain service stability and positive outcomes. Requires 18+ years old, valid Arizona driver’s license with reliable transportation, and 2+ years in relevant fields; bilingual English/Spanish strongly preferred; in-person work at the Mesa, AZ location, Monday–Friday 8:00 AM–5:00 PM with occasional evenings/weekends. Required Qualifications
- Must be at least 18 years old
- Valid Arizona driver’s license, reliable transportation, and current auto insurance
- 2+ years in healthcare, care management, care coordination, social services, or a related field (preferred experience with DDD, ALTCS, HCBS or similar programs)
- Strong communication skills with the ability to build trust with caregivers, members, and their families
- Highly organized, excellent time management, and problem-solving abilities
- Ability to manage multiple priorities while maintaining attention to detail
- Commitment to professionalism, confidentiality, and HIPAA compliance
- Bilingual in English and Spanish strongly preferred
