Job Summary
Fully remote Care Coordinator role supporting patients in the Pacific Time Zone (8:30a–5p PST). Collaborate with an interdisciplinary team (Nurse Practitioners, Nurse Care Managers, Dietitians, Pharmacists, Licensed Clinical Social Workers, Population Health Social Workers and Psychiatrists) to coordinate comprehensive patient care, focusing on Social Determinants of Health, resource navigation, and patient advocacy. Engage patients via telephonic outreach, develop and implement care plans under supervision, facilitate transitions of care across dialysis centers, identify and address barriers to care, maintain timely documentation, and educate patients and families to empower self-management. Requires strong communication, remote-work readiness (high-speed internet, dedicated quiet workspace), and ability to operate within a value-based care model. Compensation ranges from $19.00 to $23.00 per hour with eligibility for a bonus; benefits include PTO, holidays, medical/dental/vision insurance, HSA, 401(k) match, and parental leave.
Required Qualifications
- 3 years of experience in health care coordination, community health work, or similar healthcare related field
- Experience with team-based approach to care
- Excellent working knowledge of community resources
- Exceptional ability to prioritize and task-switch to meet needs of patients and overall care team
- Self-directed and motivated to resolve issues professionally and creatively
- Strong communication and teaching skills
- Intermediate skills with MS Office Suite of products including Outlook and Teams
- You have the ability to work effectively in a primarily remote environment
