Care Coordinator

Review existing patient data and information during the inpatient treatment intake process. Verify insurance validity, coordinate transportation for inpatient and outpatient services, and obtain referrals from other facilities. Collaborate with staff and providers to schedule appointments and manage work queue issues, recall lists, and waitlists. Assist in obtaining medical records and files for provider review while maintaining infection control and safety guidelines. Operate multi-line telephones and computers within an office or inpatient facility environment. Serve all persons served by OVP Health and OVP Health Care, including pediatric, adolescent, adult, and geriatric populations.
