Job Summary
Care Coordinator responsible for service linkage and care coordination for children, youth, and families with significant behavioral health needs across home, school, and community. Build and maintain relationships with family teams and community resources, ensure services align with care plans and safety/treatment needs, document activities, participate in program evaluation, and maintain required trainings (e.g., High Fidelity Wraparound, MI, Cultural Humility). Role emphasizes collaboration, culturally responsive practices, and support across child-serving systems to promote healthy outcomes.
Required Qualifications
- Experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or related public sector human services or behavioral healthcare field
- Knowledge and experience in Hi-Fidelity Wraparound preferred (Certification provided at time of employment)
- Two years of experience in a coordinated supportive services or care coordination role preferred
- Experience working with people with autism spectrum disorders and developmental disabilities preferred
- Must have Valid Driver's License
- Ability to operate, receive and conduct video calls and/or meetings in an automated office environment is required
- Ability to maintain a minimum of 100mbps for high-speed internet connection is required
- Be culturally humble or responsive with training and experience to manage complex cases
- Excellent organizational skills with the ability to stay focused and prioritize multiple tasks
- Willing to participate in and lead cross-systems care coordination
- Demonstrates a high degree of cultural awareness
- Experience with multi-need individuals and families
- Broad knowledge of community service systems
- Be able to engage families in care planning across home, school, and community
