Job Summary
Care Coordinator role in Guernsey County, OH, focused on service linkage and care coordination for children, youth, and families with behavioral health needs. Responsibilities include coordinating family-based services across home, school, and community; engaging with family teams to ensure appropriate, safe, and effective care plans; developing partnerships with community resources; maintaining documentation and participating in program evaluation and planning meetings; upholding cultural humility and wraparound practices; and ensuring families have a voice in care planning. Compensation includes a hourly pay range of $20.19-$25.03 based on experience, education, and licensure, with comprehensive benefits and a $3,000 hiring bonus.
Required Qualifications
- Experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or related public sector human services or behavioral healthcare field: three years with a high school diploma or equivalent; or two years with an associate degree or bachelor's degree; or one year with a master's degree or higher
- Knowledge and experience in Hi-Fidelity Wraparound preferred (Certification provided at time of employment).
- Two years of experience in a coordinated supportive services or care coordination role preferred.
- Experience working with people with autism spectrum disorders and developmental disabilities preferred.
- Be culturally humble or responsive with training and experience to manage complex cases
- Have the qualifications and experience needed to work with children and families who are experiencing serious emotional disturbance (SED), trauma, co-occurring behavioral health disorders, and who are engaged with one or more child-serving systems (e.g., child welfare, intellectual and developmental disabilities, juvenile justice, education)
- Excellent organizational skills with the ability to stay focused and prioritize multiple tasks
- Willing to participate in and lead cross-systems care coordination
- Able to effectively communicate through verbal/written expression
- Must be able to operate in an Internet-based, automated office environment
- Valid Driver's License required
- Ability to operate, receive and conduct video calls and/or meetings in an automated office environment is required
- Ability to maintain a minimum of 100mbps for high-speed internet connection is required
