Job Summary
Conduct service linkage and care coordination for children, youth, and families with significant behavioral health needs by identifying care requirements, defining cultural factors, and coordinating family-based services across homes, schools, and communities. Build positive relationships with families and community agencies to develop collaborative partnerships that ensure the least restrictive, most appropriate services meet safety and treatment needs. Maintain necessary documentation, participate in team and program planning meetings, and acquire knowledge of community resources while remaining current on training requirements such as High Fidelity Wraparound and Cultural Humility.
Required Qualifications
- Experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field
- three years with a high school diploma or equivalent
- two years with an associate degree or bachelor's degree
- one year with a master's degree or higher
- Valid Driver's License
- Ability to operate, receive and conduct video calls and/or meetings in an automated office environment
- Ability to maintain a minimum of 100mbps for high-speed internet connection
- Must be able to operate in an Internet-based, automated office environment
- Able to effectively communicate through verbal/written expression
- Have the qualifications and experience needed to work with children and families who are experiencing serious emotional disturbance (SED), trauma, co-occurring behavioral health disorders, and who are engaged with one or more child-serving systems (e.g., child welfare, intellectual and developmental disabilities, juvenile justice, education)
- Excellent organizational skills with the ability to stay focused and prioritize multiple tasks
- Demonstrates a high degree of cultural awareness
- Experience with multi-need individuals and families
- Broad knowledge of community service systems
- Willing to participate in and lead cross-systems care coordination
Desired Qualifications
- Knowledge and experience in Hi-Fidelity Wraparound preferred (Certification provided at time of employment)
- Two years of experience in a coordinated supportive services or care coordination role preferred
- Experience working with people with autism spectrum disorders and developmental disabilities preferred
- Experience in one or more of the following areas: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, child development
- Be culturally humble or responsive with training and experience to manage complex cases
