Job Summary
Under direct supervision, supports Care Management (CM) team by providing short-term care coordination and connection to resources for program members. Duties include administrative support to CM, collecting data for Health Risk Assessments (HRA), screening for eligibility and benefits, referring members without a PCP to Member Services, prioritizing CM services, coordinating transition of care with PCPs and medical providers, documenting encounters in electronic records, preparing monthly reports, promoting member engagement, arranging PCP visits, assisting members in accessing medical homes and social/services, and facilitating access to community resources (housing, food, clothing, prenatal classes, parenting, and mental health services). Requires collaboration with diverse teams, strong interpersonal skills, knowledge of medical terminology, and proficiency with Microsoft Office. Education and experience specify a high school diploma or equivalent and 2+ years of Medicaid population experience and OB/GYN exposure; organization emphasizes inclusivity and DEI.
Required Qualifications
- High school diploma or equivalent
- 2+ years serving the Medicaid population, preferably in the MD/DC area
- 2+ years OB/GYN experience
- Knowledge of medical terminology
- Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services
- Knowledge of transportation and other barriers to care that members may face
- Ability to communicate medical information to health care professionals and members of the care management team
- Computer Literate (Microsoft Office)
