Job Summary
Provide specialty service care coordination to members, ensuring appropriate services are delivered by specialty providers. Responsible for developing the Person-Centered Support Plan (PCSP) and coordinating with healthcare providers for diagnostics and ambulatory care. Assist members with healthcare needs, social determinants of health, health education, and comply with Conflict Free Case Management rules. Required skills include excellent communication, multi-tasking, and the ability to analyze medical and clinical information.
Required Qualifications
- High school diploma or equivalent
- Bachelor’s degree in social science or health-related field or 1+ years of experience caring for developmentally or intellectually disabled or behavioral health clients
- Valid driver's license and automobile insurance
