Care Coordinator - New Mexico ALB

Job Summary

Coordinate specialty medical referrals and assist members with navigating Medicaid Managed Care Organizations and healthcare benefits. Maintain monthly member contacts to monitor progress toward care plan goals and complete annual Comprehensive Needs Assessments. Provide individual interventions, skills building, and crisis response support while participating in multi-disciplinary team approaches. Deliver culturally relevant, person-centered services and document all activities meeting regulatory standards. Requires valid driver's license, on-call availability, and experience with behavioral health populations.

Required Qualifications

  • Valid drivers' license
  • Meets the driving privileges guidelines established in HR Policy 12.6 Vehicle Safety and Client Transportation
  • Master's Degree in a human service-related field
  • Bachelor's degree in a human service-related field
  • 1-year relevant experience working with the target population
  • High School Diploma or GED
  • Minimum of 3 years of experience working with the target population
  • Certified Peer Specialist (CPS)
  • Certified Family Specialist (CFS)
  • Ability to manage time to meet all deadlines
  • Ability to recognize and identify symptoms of mental illness and suicide and risk potential
  • Ability to respond appropriately to symptoms of mental illness and suicide and risk potential
  • Good verbal and written skills
  • Excellent organizational skills
  • Effective use of time management
  • Ability to recognize and assess suicide potential and risk
  • Ability to provide appropriate responses in emergency/crisis situation
  • Ability to recognize and identify symptoms of mental illness and substance use
  • Ability to demonstrate the ability to engage and develop rapport with individuals presenting with a variety of symptoms/presentations
  • Ability to provide services that are person-centered, culturally relevant, and are non-judgmental and welcoming to diverse populations
  • Ability to document all services provided within expected time frames
  • Ability to meet service definitions and regulatory standards
  • Ability to meet quality improvement goals
  • Ability to document all services provided in a manner that meets service definitions, regulatory standards, and agency expectations
  • Ability to consistently meet established client care measure expectation
  • Ability to participate in and complete all required training courses in an accurate and timely manner
  • Ability to attend and participate in supervision sessions
  • Ability to demonstrate the ability to apply supervisory recommendations to work performance
  • Ability to provide in on-call and crisis response/support responsibility as assigned

Desired Qualifications

  • Bachelor's degree in a human service-related field and 1-year relevant experience working with the target population preferred
  • Bilingual (English/Spanish) is preferred but not required
