Care Coordinator - Neurodevelopmental Clinic

Job Summary
Coordinate services for children and families referred to the Development Clinic by communicating with all parties involved in treatment, identifying needs, and facilitating referrals. Attend collateral meetings, maintain appropriate records of progress, and develop treatment plans in conjunction with families and referral sources. Provide community-based skills training and development services, offer emergency counseling and crisis intervention when necessary, and advocate for families with complex needs. Submit reports according to established time frames and participate in ongoing staff training and development activities. Assist with low barrier access to services and screen for appropriate community agency referrals.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, Sociology, Criminal Justice or other related degree
- Valid driver's license
- Auto insurance that meets the Indiana State minimum coverage requirement
- Qualify for Company's vehicle liability insurance
- Pass required background checks including county/state checks, fingerprinting, CPS check, sex offender registry check, and drug screen
Desired Qualifications
- Experience working with children who have a Dual Diagnoses of both Serious Emotional Disturbance and Neurodevelopmental Disorder
