Job Summary
The Care Coordinator is responsible for meeting patients face-to-face to assist in the facilitation of recovery services, ensuring proper documentation of recovery activities, and conducting monthly Care Coordination meetings. The role involves collaboration with healthcare practitioners prescribing treatment for opioid use disorder and completing required documentation. Candidates should have a bachelor’s degree in a relevant field along with experience in substance use disorders, or hold licensure as a Registered Nurse with direct experience in substance use disorder services.
Required Qualifications
- Bachelor’s degree in Social Work, Psychology, Psychiatric Rehabilitation, Sociology, Counseling, Vocational Rehabilitation, or Human Services
- One year of experience with individuals with substance use disorders or clinical experience with co-occurring diagnoses
- Licensure as a Registered Nurse with direct experience in substance use disorder services
- Board of Counseling Certified Substance Abuse Counselor (CSAC) or CSAC-Assistant
