Job Summary
Coordinate care for youth and families by facilitating admission and discharge meetings, obtaining legal signatures, and creating client-centered service plans that comply with state and federal laws. Conduct regular face-to-face and virtual contacts to review progress, maintain continuity of care, and ensure linkage with mental health and substance abuse treatment services. Establish relationships with community resource providers, schools, and stakeholders to identify service gaps and improve delivery. Maintain accurate electronic and paper case files, complete required documentation, and participate in weekly supervision and case staffings. Collaborate with the Family Peer Support Specialist to support client goals and safety.
Required Qualifications
- Master's degree preferred in a Human Service discipline from four-year accredited college or university
